Please fill out the Membership Application completely. Please make SEPARATE CHECKS for classes and membership. Mail membership checks to Membership Chairperson and give class fee checks to Class Monitors when registering at the first class if not pre-registered.

Pay class fees directly to the Class Monitor at or prior to the first class with this completed registration form. Class sizes are limited. Please contact Class Monitor prior to the first class for information and to insure your place in the class. Please fill out a new form for each session.
